Switching to Foundayo from Mounjaro or Wegovy: what the evidence says
Tired of injections, or eyeing a potentially cheaper daily tablet? Switching from an injectable GLP-1 to Foundayo is exactly what one of its major trials studied, so for once there's real evidence rather than guesswork. Here's what it showed, and how switches actually work.

Why people consider the switch
Four reasons dominate: needle fatigue, because a daily tablet suits some people better than a weekly injection ever will; travel and lifestyle, since tablets need no fridge or sharps disposal; cost, with the tablet expected to undercut injectables (UK pricing pending on our price lists); and maintenance, where someone who has lost their weight on an injectable wants a simpler way to keep it off.
What ATTAIN-MAINTAIN actually showed
The ATTAIN-MAINTAIN trial studied precisely this scenario: adults who had lost weight on injectable tirzepatide (Mounjaro's active ingredient) or semaglutide (Wegovy's) were switched to daily orforglipron. The headline finding was strong maintenance of the weight already lost, which matters because the well-documented alternative, stopping GLP-1 treatment entirely, typically sees a substantial portion of lost weight return. In other words, the tablet has trial evidence as a landing spot after injections, not just as a starting medicine.
The honest caveat: maintenance is not the same as matching an injectable's full horsepower. Mounjaro's trials showed larger average losses than orforglipron's, so someone mid-journey who switches may find the tablet holds their progress rather than accelerating it. Whether that trade suits you depends on where you are and what you want next, which is precisely the prescriber conversation.
How a switch works in practice
- It's prescriber-led, full stop. The timing of your last injection, your starting Foundayo dose and how fast you climb the ladder are individual clinical decisions, not forum wisdom.
- Never overlap. Foundayo must not be combined with any other GLP-1 medicine. A switch is a clean handover, not a transition period of both.
- Don't assume you start at the top. Even coming from a high injectable dose, expect to join the Foundayo ladder at a dose your prescriber chooses and step up on the standard schedule. The six strengths run 0.8 mg to 17.2 mg with at least a month per step; our taking Foundayo guide has the details.
- Expect an adjustment window. Gut side effects cluster around any change, switches included. The usual toolkit applies: small meals, steady fluids, patience, and a word with your prescriber if it's rough.
- Switch with the same provider where you can. A pharmacy that already holds your history makes the clinical handover cleaner. If you're changing provider too, take your treatment record with you and check the new pharmacy on the GPhC register first.
Switching the other way, or trying tablets first
The same logic runs in reverse: someone starting on Foundayo who wants more effect later can discuss moving to an injectable, and our sister sites carry live pricing for those conversations: Monj for Mounjaro and iGovy for Wegovy. And if you're weighing Foundayo against the other daily tablet, the Wegovy pill with its empty-stomach rules, Ogovy covers it, while our comparison table lines all four up.
The one-line summary
Switching to Foundayo after injections has genuine trial evidence behind it for maintaining weight loss, but every switch is a prescriber-led, no-overlap, restart-the-ladder process, and anyone promising you otherwise is selling something.
